Insurance Plans I Accept

I am paneled with the following insurance plans and will submit claims (i.e., formal requests to your insurance company for our sessions to be covered). You will be responsible for paying towards coinsurance/a deductible or a copay (whichever applies to you), and keeping me updated if your insurance plan changes.

Payment Options

I accept payment via debit or credit card (stored and automatically charged at the end of each day, with your consent), check, cash, or Health Savings Account (HSA).

Out-of-Network Reimbursement

Below are my out-of-pocket (i.e., without insurance coverage) fees:

  • 1st session (50-55 minutes): $200

  • 2nd & subsequent sessions (50-55 minutes): $175/session

You can check to find out if your insurance company offers out-of-network mental health benefits to see if you may be able to get reimbursed for part of the session fee. Below are a list of questions you can ask your insurance company:

  1. Am I eligible for out-of-network mental health benefits?

  2. What amount would I be reimbursed for the following services with a licensed psychologist:

    • CPT Code 90791

    • CPT Code 90837

  3. Do I have a deductible that must be met each year?

  4. Is there a session limit per year?

  5. Do I need preauthorization prior to receiving services? If so, what information is needed?

  6. Is approval required from my primary care physician?

  7. Is there a specific form to complete, and where/how do I obtain this form?

After you receive confirmation of your out-of-network benefits, I can provide you with a superbill (i.e., a statement of services and associated fees) for you to submit to your insurance company to receive reimbursement.

Please Note: If using your insurance plan’s out-of-network mental health benefits, you will be responsible for verifying your benefits, submitting your superbills, receiving reimbursement payments, and paying the out-of-pocket fee ($175) each session.

No Surprises Act: Psychologists and other healthcare providers are required by law to give uninsured and self-pay patients a Good Faith Estimate of costs for services that they offer.
